  Khaavren didn't say "gya" because he knew Vlad was male, and "gya"
is used (POSSIBLY AMONG
 OTHER USES) when sex is unknown.  It
is not the only pronoun used by Dragaerans, merely the only one we've
seen untranslated.

    After Khaavren said "?" = he, because he knew Vlad was male, he
then said more because he wanted to avoid any possible confusion
between two males, not between a male and a female.  He didn't know
the physician was female, and so needn't have said etc. etc.
